"OMI" DIY by Munky King (for Toysrevil Turns 8 Facebook Contest)

I had always wondered about the OMI form from Munky King, and thought to include a DIY white-blank for the erstwhile artist in you! As well this is the first time I had actually held the OMI form in my own hands as well - so thanks to the contest, I could! Good luck to you folks for joining the contest tho!
